The Familylinks Options Care Management program is a non-direct service that assists older adults ages 60 and over and their families through advocacy and referral to supports in the community. Care Managers assess, coordinate and monitor services for participants. The desired outcome of this program is to keep participants in their homes as long as they are healthy and safe.
INTERN POSITION DESCRIPTION:
This internship provides an opportunity to learn about the responsibilities and essential functions of care management. It involves learning about the ongoing needs of older adults and their families and how to navigate through the Aging network. The intern will gain experience working with older adults and will be provided opportunities to interact with participants, families, providers, and other parties involved in the Aging network. The intern will learn about the various resources available to older adults and participate in trainings offered at the AAA (Area Agency on Aging) and through Familylinks. The intern will learn about completing in-home assessments and documentation of information in SAMS (Social Assistance Management System). The intern will have opportunities to accompany Care Managers on home visits and observe completion of in-home assessments. The intern will also assist with completing monthly phone contacts. Special projects may be available for the intern to assist with depending on program needs. The intern will participate in regular supervisions and team meetings.
The intern must be enrolled in an undergraduate program in Social Work or a Social Service related field. Act 33/34 and FBI clearances are required. The intern must possess a valid driver’s license and access to a vehicle.
Normal business hours are 8:30 a.m. to 4:30 p.m. Monday through Friday at the Forest Hills Office.
