Note: Options Care Management serves participants referred by Allegheny County’s Area Agency on Aging. To enroll, you must first contact the county’s Seniorline. Familylinks does not accept direct referrals to join the Options program.
As one grows older, some tasks become more of a challenge — but moving into an assisted-living facility or living with friends or family isn’t always the most desirable choice. The Options program is a statewide initiative to help older adults who wish to remain as independent as possible. Options Care Management provides you with a care manager to ensure you receive the services you need in order to live independently in your home.
Who is eligible?
Allegheny County residents, age 60 and over, who require help remaining independent and living in their own homes.
What can you expect?
A care manager (CM) is assigned to each participant. The CM coordinates care and services that are available to the client, such as:
- The Family Caregiver Support Program
- Home-delivered meals
- Senior Companion Station
- Links to Options-contracted service providers
- Links to community resources as needed
The CM and participant are in touch monthly via telephone. Options also provides resources to families and caregivers working with the client.
How do you enroll?
Options participants are referred through Allegheny County’s Seniorline. If you or someone you know might qualify, contact the county’s Department of Human Services at 412-350-5460; if care management is recommended, you will be referred to Familylinks or another local provider based on location.
