Service Coordination

Service Coordination is a non-direct service that assists children and adults and their families through advocacy and referral to necessary supports in the community.  Care Managers and Supports Coordinators are required to locate, coordinate, and monitor services for individuals assigned to their caseloads.   The goals of our service coordination programs are 1) To conduct timely and comprehensive assessments of consumers’ and family’s needs; 2) To identify service needs through the development of Individual Support Plans; and, 3) To provide choice of service interventions and/or links with appropriate Familylinks or community services.
